Local Application of Human Fibrin Seal (Tissucol/ Tisseel) in Tonsillectomies and Adenectomies in Patients with Bleeding Disorders. Ten Years’ Clinical Experience

Abstract

Clinical experience with a thrombin-converted fibrin seal in tonsillectomies and adenectomies in 272 patients with different bleeding disorders during a time span of 10 years is reported. In all these cases a complete primary hemostasis due to the fibrin seal, applied to an allogenic collagen implant, could be obtained without any substitution of the deficient blood coagulation factors. The fibrin seal has highly adhesive properties and strong resistance against proteolytic activities, and the sealed wounds showed an optimal healing tendency. Moderate postoperative hemorrhages resulted in only four cases of hemophilia but these stopped immediately after substitution of a single dose of either factor VIII or IX concentrate. Encouraged by this excellent results, further in 44 patients with severe hemophilia the local wound sealing with the fibrin glue following tonsillectomies and adenectomies was combined with a low-dose substitution therapy with clotting factor concentrates, i.e., the deficient factor level in the blood was raised to approximately 10% for 10 days. In these cases no postoperative hemorrhage resulted.
