Zusammenfassung
Das vordergründige therapeutische Ziel beim ARDS besteht derzeit darin, die schwere Hypoxämie wie auch die metabolische und respiratorische Azidose mit Hilfe der Beatmungstherapie unter Anwendung einer erhöhten inspiratorischen O2-Konzentration sowie mit Puffertherapie zu behandeln. Einige dieser Patienten, die unter dieser Behandlung keine Verbesserung erkennen ließen, wurden mittels extrakorporaler Membranoxygenierung (ECMO) [24] oder mit extrakorporaler CO2-Ausscheidung in Verbindung mit einer “low frequency Ventilation” behandelt [4]. Die erste der genannten Oxygenierungsmethoden war klinisch nicht erfolgreich [9, 16], wogegen die Ergebnisse mit der letztgenannten Methode besser sind [4]. Die personellen und technischen Voraussetzungen zur Durchführung der letztgenannten Methode sind jedoch so groß, daß sie für eine breitenwirksame Anwendung wahrscheinlich keine Bedeutung erlangen wird. Auch die Transplantation der Lunge hat bis heute keine klinische Bedeutung erlangt [23], so daß die Mortalität bei ARDS-Patienten mit über 50% in den letzten 2 Jahrzehnten unverändert hoch geblieben ist.
