Zusammenfassung
Wenn man das Pathologische am Wasser- und Elektrolythaushalt erkennen und behandeln will, muß man sich die physiologischen Veränderungen, die bereits die normale Schwangerschaft verursacht, vor Augen halten. Denn es ist physiologisch, daß die Gravida Wasser retiniert, bis ihr intravaskuläres Volumen um etwa 40% (im Schnitt 1, 5 1) und ihr extrazelluläres Wasser um etwa 30% (41) erhöht ist. Das gleiche gilt für die Elektrolyte. Parallel zum Wasser ist besonders auch die Vermehrung des Gesamtkörpernatriums notwendig. Im Durchschnitt erhöht sich dieses um 750 mmol (15,5 g). Daß das Retinieren und Ausscheiden von Wasser und Natrium Hand in Hand gehen, ist hinreichend bekannt. Bei Schwangeren ist jedoch das Verhältnis vom Gesamtkörperwasser zum Gesamtkörpernatrium größer als bei Nichtschwangeren. Dies ist daran erkennbar, daß das Serumnatrium, trotz gesteigerter Retention, mit durchschnittlich 135 mMol/1 unter der Norm der Nichtschwangeren liegt. Daß unter diesen Umständen leichte Ödeme normal sein können, ja manche Autoren in diesen einen Schutzfaktor zu erkennen glauben [4, 25, 32], erscheint daher nicht widersprüchlich.
