Der Risikopatient in der Anaesthesie

Zusammenfassung

Herzinsuffizienz, koronare Herzkrankheit und hypertensive Krisen gehören — neben spezifischen Herzmuskelerkrankungen, primären Kardiomyopathien sowie angeborenen und erworbenen Herzerkrankungen — zu den gravierenden prä- und perioperativen Risikofaktoren. Eine Herzinsuffizienz ist dadurch gekennzeichnet, daß das Herz als globales Pumporgan nicht in der Lage ist, eine ausreichende nutritive Organdurchblutung für die kardialen und extrakardialen Stromgebiete aufrechtzuerhalten. Ätiologisch kommen koronare, myokardiale und extrakardiale Ursachen in Betracht, am häufigsten ischämische und hypertensive Herzerkrankungen sowie primäre und sekundäre Kardiomyopathien (spezifische Herzmuskelerkrankungen).
