Abstract
Since BEHAR (1) first introduced epidural morphine for treatment of pain, morphine solution has generally been administered in a high injection volume. Yet epidural morphine in low injection volumes is also effective in the treatment of acute (3, 2) and chronic (4, 8) pain, as we were able to prove in 1984 (6). Recently we have demonstrated that cisternal cerebellomedullary CSF concentrations of morphine are dependent on the volume of the epidural injection; this suggests that the risk of respiratory depression might be reduced with low-volume injections of epidural morphine (5), as HUG et al. demonstrated in 1981 that there is an evident relationship between cisternal cerebellomedullary CSF concentrations and end-tidal CO2 (10).
