Zusammenfassung
Es wird geprüft, inwieweit der unterschiedliche Exzisionsabstand bei primären malignen Melanomen der Haut gravierenden Einfluß auf die Überlebenswahrscheinlichkeit der Patienten ausübt. 44 Melanome an Extremitäten und Rumpf, die in den Jahren 1973 bis 1978 weit exzidiert worden waren (kleinster Abstand vom Tumorrand bis zur Exzisionsstelle 3 cm und mehr), wurden ausgewertet. Die Überlebenskurven dieser Patienten wurden verglichen mit denen von 44 Symptomzwillingen gleichen Geschlechts, die ein Melanom gleicher Dicke, gleicher Ulzeration und gleicher Lokalisation hatten. Diese Patienten stammen aus der Arbeitsgemeinschaft „Malignes Melanom“1. Die Tumoren waren mit einem Exzisionsabstand bis maximal 2,0 cm operiert worden. Es zeigt sich, daß bei dünnen Melanomen (\( \underline \angle\) 1,5 mm) die Prognose der Patienten durch die weite Exzision nicht verbessert wird (6 Jahres-Überlebensrate jeweils ca. 85%). Bei Tumoren größerer Dicke (>1,5 mm) jedoch überleben signifikant mehr Patienten (p = 0,0004) mit einer weiten Exzision als solche mit knappem Exzisionsabstand.
