Angeborene Riesennävi: Prognose und Therapiemöglichkeit

Zusammenfassung

Bei kongenitalen Riesennävi handelt es sich um Extremvarianten der bei ca. 1% der Neugeborenen bereits vorhandenen Pigmentnävi. Aufgrund ihrer erhöhten Potenz zu maligner Entartung liegt bei den großen kongenitalen Nävi eine absolute Indikation zur operativen Entfernung vor. Darüber hinaus sollten auch solche angeborene Naevi pigmentosi chirurgisch entfernt werden, die aufgrund ihrer Lokalisation und Oberflächenstruktur kosmetisch störend wirken oder suspekt auf eine maligne Transformation sind.

Im frühen Kindesalter ist die hochtourige Dermabrasion großer kongenitaler Nävi bei kritischer Anwendung und subtiler Operationstechnik die Behandlungsmethode der Wahl. Sie erlaubt die vollständige Entfernung der Fehlbildung ohne wesentliche Narbenbildung. Im späteren Lebensalter, bei Rezidiven nach wiederholten erfolglosen Dermabrasionen und bei Melanophakomatosen muß der Nävus exzidiert und der Defekt plastisch-chirurgisch versorgt werden.
