Zusammenfassung
Der klassische neurochirurgische Zugang zu thorakalen Bandscheibenvorfällen ist die komplette oder partielle Laminektomie, die nach lateral durch Wegnahme der Gelenkfortsätze und Entfernung der Bogenwurzel erweitert werden kann (Patterson u. Arbit 1978). Generell haben sich aber diese Zugänge nicht bewährt, da sie mit einer erhöhten Gefährdung des Markes einhergehen (Perot u. Munro 1969). Zum Beispiel werden 33% postoperative Verschlechterungen bei thorakalen Bandscheibenoperationen mit dorsalen Zugängen angegeben (Benjamin 1982). Aus diesen Gründen hat man Alternativzugänge entwickelt, die ein mehr laterales oder ventrales Vorgehen zur Wirbelsäule gewährleisten. Am häufigsten werden ein dorsolatera-ler, extrapleuraler Zugang nach Kostotransversektomie (Hulme 1960) und ein ventrolateraler, transpleuraler nach Thorakotomie (Crafoord 1968, Ransohoff et al. 1969) verwendet.
