Zusammenfassung

Dysphagie definiert man als die Schwierigkeit, flüssige oder feste Nahrung hinunterzuschlucken. Sie ist Folge einer positionsunabhängigen, zwischen Mundhöhle und Magen gelegenen Passagebehinderung. Dysphagie tritt lediglich während eines Schluckakts auf, d. h. dieses durch die veränderte oder verzögerte Nahrungs oder Flüssigkeitspassage hervorgerufene Gefühl tritt innerhalb von 10 s nach Beginn des Schluckakts auf. Sobald der Schluckakt in seiner Komplexität gestört ist, wird sich der Patient dieses ansonsten kaum beachteten Prozesses bewußt, und dies wird ihn veranlassen, ärztliche Hilfe zu suchen. Er kann das Gefühl haben, nicht mehr schlucken zu können, weil die Zunge nicht mehr imstande ist, den Bolus in den Hypopharynx zu befördern, weil Flüssigkeit durch die Nase zurückströmt oder in die Lunge aspiriert wird. Andererseits kann die Schluckschwierigkeit aus einem retrosternalen Druckoder Engegefühl bestehen, wobei die Nahrung entweder nur kurz oder für längere Zeit stecken bleibt. Dysphagie kann mit Schmerzen verbunden sein, sollte aber nicht mit Odynophagie verwechselt werden. Darunter versteht man schmerzhaftes Schlucken ohne Verzögerung der Nahrungspassage.
