Zusammenfassung
Der therapeutischen Verwendung von Antikörpern (Immunglobulinen) liegen unterschiedliche Vorstellungen zugrunde. Bei angeborenen oder - häufiger - erworbenen Antikörpermangelzuständen hat die Zufuhr von Immunglobulinen den Charakter einer Substitutionstherapie: eine nicht oder in zu geringer Menge vorhandene Komponente des Immunsystems wird ersetzt; dadurch wird ein funktionelles Defizit behoben. In ähnlicher Weise kann ein Mangel an spezifischen, d.h. gegen einen bestimmten Infektionserreger gerichteten Antikörpern durch die Zufuhr von Antikörperpräparaten kompensiert werden, die den fehlenden Antikörper in relativ hoher Konzentration enthalten. Dies geschieht meist zu prophylaktischen Zwekken, etwa bei Verdacht auf eine erfolgte Infektion mit dem Erreger der Hepatitis B, mit dem Hepatitis A-Virus, mit dem Tollwuterreger oder mit Clostridium tetani. Die Zufuhr spezifischer Antikörper kann gelegentlich auch in therapeutischer Absicht erfolgen, z. B. bei der Behandlung eines durch gramnegative Bakterien hervorgerufenen Endotoxinschocks durch Antikörper der Klasse IgG oder IgM, die gegen den Lipid A-Anteil des Endotoxins gerichtet sind. Für die genannten Beispiele ist der Zusammenhang mit der von Emil von Behring geschaffenen „passiven Immunisierung“ ohne weiteres ersichtlich [2,3,4].
