Erfahrungen mit der Lymphoszintigraphie bei Stamm-Melanomen

Zusammenfassung

Die Lymphoszintigraphie mit Tc^99mAntimonium-Kolloid hat sich in den letzten Jahren zunehmend durchgesetzt, um die Metastasierungsrichtung des malignen Melanoms am Stamm zu bestimmen. In Abhängigkeit von der Lokalisation erolgte bei 84 Stamm-Melanomen (1981–1984) die Lymphdrainage in 35% der Fälle in eine regionale Lymphknotengruppe, in 53% der Fälle in zwei (37%) oder mehr Lymphknotengruppen; in 12% der Fälle war kein Abfluß nachweisbar. Bei 84% der Stamm-Melanome entsprach die Lymphabflußrichtung dem Sappey-Schema. Bei 7 Patienten mit Stamm-Melanomen trat eine Metastasierung in den regionalen Lymphknoten auf; bei 5 Fällen (71%) war diese Lymphknotengruppe richtig vorausbestimmt worden. Bei 3 weiteren Patienten entsprach die Richtung später auftretender Transitmetastasen der lymphoszintigraphisch vorausbestimmten Lymphdrainage in die regionale Lymphknotengruppe.

Summary

Lymphoszintigraphy with ⁹⁹Technetium-antimony-colloid was established during the last years to predict the direction of lymphogenic metastases in truncal melanoma. Depending on the primary localisation of the tumor in 35% of 84 cases ^99mTc-AC was drained exclusively to one nodal group. In 53% of our cases we identified two (37%) or even more endangered groups of lymphatic nodes. 12% of our patients showed no evident drainage of intradermal injected ^99mTc-AC near the primary site of the tumor. In 84% the lymphatic drainage correlated with Sappey’s lines. 7 melanomas actuary metastasized in regional lymph node-groups, in 5 cases (71%) the predicted region was affected. Three other patients suffered from transit metastases near the localisation which had been marked by ^99mTc-AC.
