Zusammenfassung
Zu Beginn wird auf die Bedeutung der Skelettszintigraphie mit 99mTechnetium-Phosphat-Verbindungen und die der Gelenkszintigraphie mit 99mTechnetium-Pertechnetat eingegangen und ihre Vorzüge gegenüber der klinischen und röntgenologischen Erfassung von Knochen- und Gelenkveränderungen herausgestellt. Die Anwendung dieser beiden Szintigraphieformen, vor allem der Skelettszintigraphie, brachte an einem unselektierten dermatologischen Krankengut von Psoriasispatienten neue Erkenntnisse über Häufigkeit, Ausprägung und Wertigkeit der psoriatischen Osteoarthropathie. So konnte gegenüber einer klinisch- und röntgenologisch-erfaßbaren Inzidenz von bis zu 7%, szintigraphisch eine solche von maximal 90% nachgewiesen werden. Dies gelang durch die bessere Registrie-rung ihres polytopen Charakters und den Nachweis von potentiell reversiblen, größtenteils klinisch stummen und röntgenologisch nicht erfaßbaren „Frühformen“. Daneben wurden auch gelenkferne Manifestationen einer psoriatischen Osteopathie erfaßt, die vor allem Schädel, große Röhrenknochen, Rippen und auch Endphalangen betreffen. Ein Entwurf zur formalen Pathogenese der psoriatischen Osteoarthropathie wird aufgrund der szintigraphischen Befunde in Verbindung mit pathologisch-anatomischen Befunden gemacht.
Zahlreiche Fragen zur Aussagekraft der Szintigraphie vor allem im Hinblick auf die durch sie mögliche Abgrenzung der psoriatischen Osteoarthropathie von der chronischen Polyarthritis (Frühformen, Befallmuster etc.) und der Reproduzierbarkeit bzw. Bestätigung der alten früheren Befunde werden an den Nuklearmediziner gerichtet. Höchstwahrscheinlich besitzt jede Erscheinungsform der Psoriasis im Hinblick auf Polytopie und Befallmuster ihre eigene Ausprägung.
Summary
At the beginning we discuss the importance of bone scintigrafy with 99mTechnetium-phosphate-substances and joint scintigrafy with 99mTechnetium-pertechnetate. We emphasise their advantages as opposed to clinical investigation and radiological methods of joint- and bone disease. We received new knowledge about frequency, dimension and validity of psoriatic osteoarthropathia by using scintigrafic methods, especially bone scintigrafy on one group of unselected psoriatics. There is an incidence of about 7% by clinical and radiological investigations and a maximum of 90% by scintigrafy. There was a better registration of the polytopical character and proof of potentially early manifestations which were not clinically and radiologically detectable. We could see furthermore manifestations of psoriatic osteopathia not only in the joints but also in the scull, the long marrow bones, the ribs and the distal phalanges. We present a congestion about formal pathogenesis of psoriatic osteoarthropathia from scintigrafic findings in connection with histological results. There are many questions about the significance of scintigrafy to nuclear medicine: Is there a possibility to separate psoriatic osteoarthropathia from rheumatoid arthritis (early manifestations, form of manifestation)? Can the earlier results be confirmed and reproduced? Most likely every single variety of psoriasis has its own form of manifestation in reference to the polytopical manifestation and localisation.
