Zusammenfassung

Der röntgenologische Aspekt intrathorakaler Tumoren wird bestimmt durch die Interferenz von organtypischen Reaktionsweisen der Lunge wie z.B. die Ausbildung von Atelektasen oder Pneumonien und von eigenständiger Tumordynamik. Die eigenständige Tumordynamik drückt sich in Wachstums- und Ausbreitungsform der Tumoren aus: Die lokal aggressive Tumorgruppe hat viel eigenes Stroma und neigt zu Tumorschrumpfung, zu Kavernisierung und zu Ausbrechen in angrenzende Strukturen. Die Gruppe der schnellwachsenden, früh lymphogen und hämatogen metastasierenden Tumoren nutzt dagegen das Lungengrundgerüst zu mehr appositionell-spreitendem Wachstum, bildet kaum Tumorkavernen aus und bricht seltener in angrenzende Strukturen aus. Reaktionsweise der Lunge und Tumoreigenheiten sind also Beurteilungsgrundlage. Für die Indikationsstellung und Beurteilung von Röntgenuntersuchungen ist daher eine funktionelle und räumliche Betrachtungsweise absolut notwendig. Die sich daraus ergebende Bedeutung von Röntgenuntersuchungen in der Beurteilung von intra-thorakalen Tumoren ist in Tabelle 1 zusammenfassend dargestellt.
