Zusammenfassung
Monoklonale Antikörper stellen ein wertvolles diagnostisches Mittel dar, das uns an Zellen der Lympho- und Hämatopoese eine differenzierte Analyse nach Zellreihenzugehörigkeit, Differenzierungsgrad und Funktionszuständen erlaubt. Sie setzt jedoch die Anwendung einer Reihe monoklonaler Spezifitäten voraus. Empfindlichkeit der Nachweismethode und auch die Zahl analysierter Zellen bestimmen dabei die Zuverlässigkeit unseres Ergebnisses. Die in der immunologischen Zelldiagnostik gebräuchlichste Methode ist die Immunfluoreszenzuntersuchung an Zellsuspensionen. Aufgrund ihres hohen Zellbedarfs von 0,5–1 Million Zellen für eine Antigenbestimmung kann diese Methode jedoch bei zellarmen Punktaten von Liquor oder Ergüssen ein zugleich differenziertes und verläßliches Ergebnis häufig nicht liefern. Dieses methodische Problem wird auch in einer kürzlich von Hauser et al. (1983) veröffentlichten immunfluoreszenzmikroskopischen Studie an Liquor bei multipler Sklerose deutlich. Eine zweite Begrenzung der Immunfluoreszenzmethodik besteht in der eingeschränkten morphologischen Beurteilbarkeit im Phasenkontrast, insbesondere auch in Anbetracht der raschen Fluoreszenzabnahme unter Beleuchtung. So können Monozyten und Granulozyten bei Bestimmung der T-Zellen fälschlich in der Negativfraktion oder bei Bestimmung Immunglobulin-tragender B-Zellen in der Positivfraktion mitgezählt werden, ebenso wie neutrophile Granulozyten fälschlich in der Fraktion c(ommon)-ALL-Antigen-positiver lymphatischer Zellen (Braun et al. 1983).
