Zusammenfassung
Seit Einführung von H2-Rezeptorantagonisten ist es möglich, Patienten mit pathologisch gesteigerter Säuresekretion (überwiegend mit Zollinger-Ellison-Syndron, ZES) über kurze Zeit medikamentös wirkungsvoll zu behandeln [1–3]. Die früher notfallmäßig durchgeführte totale Gastrektomie wurde somit weitgehend überflüssig. Folgestudien zeigten, daß dieses Krankheitsbild auch über einen längeren Zeitraum mit H2-Blockern beherrscht werden kann [4–9]. Allerdings ist eine Langzeittherapie mit Cimetidin nicht unproblematisch [10–14]. Während in einigen Studien mehr als 90% der Patienten mit ZES über einen Zeitraum bis zu 7 Jahren erfolgreich behandelt werden konnten, berichteten andere Autoren über eine relativ hohe Rate sog. Cimetidinversager [4, 7, 10–12, 14]. Eine Langzeittherapie wird auch dadurch kompliziert, daß bei ca. 60% der Patienten die Cimetidindosis gesteigert werden muß [4, 14]. Auch muß Cimetidin in relativ kurzfristigen Zeitabständen tagsüber und in der Nacht verabreicht werden [4, 14]. 8–50% aller männlichen Patienten mit pathologisch gesteigerter Säuresekretion entwickeln unter einer hochdosierten Cimetidintherapie Impotenz, Gynäkomastie und Brustspannung [12, 14, 16].
