Antiandrogene in der Dermatologie

Zusammenfassung

Störungen der hormonellen Regulierung des Talgdrüsenhaarfollikelapparats, entweder durch überschießende adrenale bzw. ovarielle Androgenproduktion oder durch beschleunigte Umwandlung inaktiver Steroidvorstufen zu Dihydrotestosteron, führen zu Seborrhoe, Acne, Hirsutismus und androgenetischem Effluvium.

Die therapeutische Konsequenz ist die Anwendung von den Antiandrogenen, Substanzen die den An-drogenmetabolismus in allen Endorganen mit Androgenreceptoren blocken. Zur Zeit ist Cyproteronace-tat, das den Androgenreceptor kompetitiv hemmt, das stärkste Antiandrogen und kann in Kombination mit Äthinyloestradiol als orales Contraceptivum angewandt werden.

In der hohen Dosierung mit 100 mg/Tag wurde Cyproteronacetat erfolgreich bei Frauen mit Hirsutismus eingesetzt, während Acne und Alopecie auch auf die niedrige Dosierung mit 2 mg/Tag in mehr als 80% befriedigend ansprechen.

Summary

Dysfunction of the hormonal control of the pilosebaceous unit caused by excessive adrenal or ovarian synthesis of androgenic hormones or increased peripheral conversion of inactive precursor steroids to di-hydrotestosterone result in disorders such as seborrhea, acne, hirsutism and androgenetic alopecia. The therapeutical consequence is the use of antiandrogens, substances blocking the metabolic steps in all target organs with androgen receptors. At the moment cyproteronacetate which competes with the binding of the androgen to its receptor is the most effective antiandrogen and can be used in combination with ethinylestradiol as an oral contraceptive.

In a high dosage of 100 mg/day cyproteronacetate was successfully used in hirsute females, while in patients with acne and alopecia a low dosage of 2 mg/day leads to an overall improvement in 80% and more.
