Zusammenfassung

Es wurden verschiedene Arten von sphinktererhaltenden Operationsmethoden beschrieben [19]. Sie unterscheiden sich hauptsächlich darin, wieviel Dickdarm im Bereich des Rektums entfernt wird. Zum einen kann das gesamte Rektum und der anale Sphinkterapparat entfernt und Kolon oder Ileum am äußeren Analring anastomosiert werden. Dies ist das Prinzip der Swenson-Operation zur Behandlung der Hirschsprung-Erkrankung [29]. Es ist auch möglich, die äußeren und inneren Sphinkteren zu erhalten, die Rektumschleimhaut zu entfernen und das obere Sigma oder das Ileum durch diesen Muskelschlauch mit einer Anastomose am Anus hindurchzuziehen; dies ist das Operationsverfahren nach Soave [27]. Das Verfahren nach D’Allaines verbindet die tiefe anteriore Präparation mit einer Anastomosierung des Sigmas ans Rektum in Höhe des Dammes; man erhält dabei den Analkanal und 3–4 cm des unteren Rektums [6]. Black beschrieb einen endorektalen Zugang, der die untersten 2–3 cm der Rektumschleimhaut und den unteren Teil des Sphinkterapparates erhält [4]. Bacon erhielt die Sphinkteren nach einer kombinierten abdominoperinealen Darstellung [1].
