Abstract

Glottic carcinoma may extend to or arise from or in the neighborhood of the anterior commissure (Figs. 1 and 2), which is defined as the anterior midline between the vocal cords measuring only 2–3 mm in height (van Nostrand 1974). There is only a thin layer of submucosa and a fibrous cord — “the anterior commissure tendon” — that separates the mucosa from the underlying cartilage in the anterior commissure area, which may explain the increased risk for cartilage invasion compared to that for tumors on the vocal cords where muscles and perichondrium intervene.
