Zusammenfassung
Im Rahmen biologisch-psychiatrischer Forschung hat in den letzten Jahren der Dexamethason-Suppressionstest (DST) erhebliches Interesse gefunden. Insbesondere die Arbeitsgruppe von Carroll hob in einer Vielzahl von Publikationen die differentialdiagnostische Wertigkeit des DST bei psychiatrischen Erkrankungen hervor. Die Ergebnisse und Schlußfolgerungen von Carroll et al. sowie von anderen, damit übereinstimmenden Arbeitsgruppen können wie folgt zusammengefaßt werden:

Nach oraler Gabe von 1–2 mg Dexamethason um 23 Uhr besteht bei gesunden Probanden und bei psychiatrischen Patienten, die nicht an einer endogenen Depression erkrankt sind, in ca. 96% der Fälle über 24 h eine Suppression des Plasmakortisols, d.h. alle Plasmawerte liegen unter 5 µ/dl [8, 14, 35, 39, 45].

Endogen-depressive Patienten sind in etwa 50% der Fälle gar nicht supprimiert oder zeigen eine vorzeitige Aufhebung der Kortisolsuppression („early escape“). Bei hospitalisierten Patienten und Gabe von nur 1 mg Dexamethason sind sogar 2/3 der endogen Depressiven nicht voll supprimiert [8, 14, 35, 39].

Das Ergebnis des DST korreliert nicht mit dem Alter der Patienten. Auch besteht keine Korrelation zum Schweregrad der Depression, doch sind Patienten mit einer produktiv-psychotischen Depression in der Regel nicht supprimiert [11, 14, 38].

Der DST ermöglicht, Patienten mit einer larvierten bzw. maskierten endogenen Depression, die das klinische Bild etwa eines Borderline-Syndroms, einer Persönlichkeitsstörung, einer Pseudodemenz oder einer Schmerzsymptomatik bieten, richtig zu diagnostizieren [7, 12, 15, 18, 32, 37].

Ein abnormer DST weist auf eine für endogene Depressionen hochspezifische limbisch-hypothalamische Funktionsstörung hin und untermauert die Indikation zu einer somatischen Therapie [8, 9, 14, 25].
