Zusammenfassung

Das EKG dient in der Klinik nach wie vor zur Erkennung und Lokalisation von frischen und alten Infarkten. Ein normales EKG in Ruhe schließt aber das Vorliegen einer schweren stenosierenden Koronarsklerose nicht aus. So haben etwa 50% der Patienten mit schwerer koronarer Herzkrankheit ein normales Ruhe-EKG. Dies gilt auch, wenn man neben den üblichen Ableitungen wie die Einthoven-Ableitungen I, II, III die Goldberger-Ableitungen aVR, aVL, aVF sowie die Wilson-Ableitungen V1–V6, Zusatzableitungen wie z.B. die Wilson-Ableitungen V1–V6 2 Interkostalräume höher bzw. tiefer oder die Ableitungen nach Nehb wählt. Die Erkennung von Infarkten wird zwar durch die Zusatzableitungen verbessert, dennoch bleibt ein großer Anteil von Patienten mit schweren Koronarveränderungen, die ein normales Ruhe-EKG aufweisen.
