Zusammenfassung

Diagnostik und Therapie der Ischämie der infratentoriellen Hirnanteile haben später begonnen als die der supratentoriellen. Das mag an einer Vielzahl von Gründen liegen: 1. Die Symptome der infratentoriellen Ischämie zeigen eine größere Variationsbreite als die der supratentoriellen, so daß sie auch häufig als „unspezifisch“ charakterisiert wurden. 2. Die für eine genaue Operationsplanung erforderliche invasive Diagnostik von Gefäßprozessen in diesem Gebiet, die Vertebralisangiographie, ist nach der Karotisangiographie entwickelt worden (Moniz 1934; Sugar et al. 1949; Lindgren 1950) und mit einer höheren Komplikationsrate behaftet als die Karotisangiographie. 3. Der Zugang zur A. vertebralis als der zuerst betroffenen Arterie ist deutlich schwieriger als zu der A. carotis. 4. Der Anteil der Patienten mit symptomatisch gewordenen, angiographisch nachgewiesenen Stenosen oder Verschlüssen der vertebrobasilären Zirkulation ist deutlich geringer als der Patienten mit entsprechenden Läsionen der Karotisstrombahn (in der „Joint Study“ 555 Patienten mit vertebrobasilären Läsionen bei einer Gesamtzahl von 2090 Läsionen; Lyons 1965).
