Behandlungsergebnisse der akuten myeloischen Leukämie beim Erwachsenen

Zusammenfassung

Fünfundsechzig unvorbehandelte Patienten (34 Frauen, 31 Männer) mit akuter myeloischer Leukämie wurden zur Remissionsinduktion mit einer Kombination aus Cytarabin (100 mg/m2/Tag kontinuierlich i.v. Tage 1–7) und Daunorubicin (45 mg/m2/Tag i.v. Tage 1–3) behandelt. Das Alter der Patienten lag zwischen 15 und 71 (Median 43) Jahren. Bei 36 Patienten ( = 55%) wurde eine Vollremission erreicht. Im Mittel wurden 1,5 Therapiezyklen und 41 Tage benötigt, um die Vollremission zu erreichen. Geschlecht und Leukämietyp schienen die Remissionsrate nicht zu beeinflussen. Das Alter ( > 50 Jahre) war ebenfalls ohne signifikanten Einfluß auf die Remissionsrate, prädisponierte jedoch zu letalen Komplikationen. Zur Remissionserhaltung wurde während des ersten Jahres alle 4 Wochen und während des zweiten Jahres alle 6 Wochen Cytarabin (200 mg/m2/Tag i.v. in 2 Dosen Tage 1–5) alternierend in Kombination mit einer der folgenden Substanzen: 6-Thioguanin (200 mg/m2/Tag p.o. in 2 Dosen Tage 1–5), Cyclophosphamid (800mg/m2 i.v. Tag 1), CCNU (100 mg p.o. Tag 1) oder Daunorubicin (45 mg/m2/Tag i.v. Tage 1–2) gegeben. Die mediane Remissionsdauer und Überlebensdauer betrugen 11 bzw. 18 Monate. Vierundvierzig Prozent der Patienten war nach einem Jahr, 28% nach zwei Jahren und 14% nach 5 Jahren noch in Vollremission.
