Abstract

In the premenopause a significant decrease of the mean ovarian estrogen production of plasma and tissue values occurs. Follicle-stimulating hormone (FSH) rises more than luteinizing hormone (LH) and accordingly the LH: FSH ratio falls. The incidence of ovulations diminishes, luteal insufficiency or absence of corpus luteum is frequent (monophasic cycle). As a consequence, endometrial hyperproliferation, anomalies of the cycle, and of menstrual bleeding may occur. Myomas become symptomatic. The incidence of curettage and hysterectomy rises. Premenstrual syndrome, mastopathy, and migraine are more common. Benign and malignant genital and breast tumors are more often found. Typical climacteric complaints (mostly of low to medium severity) occur in about 20% during the premenopause while they occur in 60%-84% during the postmenopausal period. Hot flushes are relatively rare, while irritability, nervousness, and depression are relatively more frequent. Neuroses and psychotic reactions show up more often in this age period.
