Zusammenfassung
Der objektive Nachweis einer Läsion des afferenten sensiblen Systems innerhalb des Spinalkanals außerhalb des klinischen Befundes mit dem Ziel einer topodiagnostischen Eingrenzung kann schwierig sein (Jörg et al. 1980). Neben der Myelographie, die zwar den raumfordernden Prozeß, nicht aber die tatsächlich wirksam gewordene Läsion an den neuralen Strukturen nachweisen kann, bieten sich somatosensorisch evozierte Potentiale (SEP) (Schramm et al. 1980 a, b, Schramm et al. 1982, Jörg et al. 1982, Stöhr und Dichgans 1982), die Cauda-equina Neurographie (Jörg et al. 1980), sowie die EMG-, NLG-Diagnostik einschließlich des H-Reflexes und der F-Welle an. (Ludin und Tackman 1979, Magladery 1955). Bei den somatosensorisch evozierten Potentialen (SEP) sind je nach Stimulationstechnik, d.h. Dermatom- oder periphere Nervenstimulation, unterschiedliche Aussagen möglich (Scarff et al. 1981, Zenkov 1976, Feinsod et al. 1982). Anhand eigener Befunde wird der mögliche Wert der segmentalen SEP-Untersuchungstechnik dargestellt.
