Summary
In chronic as well as in a few examined cases of acute occlusive arterial disease of the limbs with advanced ischaemia and threatening amputation a favourable effect of intravenous Venoruton® injections on the cutaneous microcirculation could be objectively assessed by fluorescein angiography (FA) in the majority of cases. An increase of blood perfusion in FA was accompanied by clinical improvement with respect to rest pain and often also to the initiation of the healing process of ischaemic lesions.
The Venoruton® treatment was throughout combined with oral anticoagulation which was continued afterwards as longterm treatment. In a special follow-up study together with the Dpt. of Vascular Surgery, Huddinge University Hospital, Stockholm, amputation could be avoided in 15 of 29 such advanced cases for an observation period of so far 28 months. The anticoagulation therapy continued as longterm treatment seems to have cooperated with the beneficial effect of Venoruton® and to have preserved it for a longer period.
Zusammenfassung
Sowohl bei chronischen als auch einigen untersuchten akuten Fällen von Arterienverschlüssen von Gliedmaßen mit vorgeschrittener peripherer Ischämie und drohender Amputation, konnte meistenteils mit der Fluoreszeinangiographie (F.A.) objektiv eine günstige Wirkung von intravenösen Venoruton®-Injektionen auf die kutane Mikrozirkulation festgestellt werden. Die mit der F.A. registrierte Perfusionssteigerung entsprach einer klinischen Verbesserung mit Abnahme der Ruheschmerzen und vielfach auch beginnender Abheilung ischämischer Läsionen.
Die Venoruton®-Behandlung wurde durchwegs mit oraler Antikoagulationstherapie kombiniert und damit auch nachher fortgesetzt. In einer „follow-up“-Studie zusammen mit der Gefaßchirurgischen Abteilung des Huddinge Universitätskrankenhauses, Stockholm, konnte bei 15 von 29 amputationsbedrohten Patienten (52%) die Amputation während einer durchschnittlichen Beobachtungszeit von bisher 28 Monaten verhindert werden. Die Antikoagulatienstherapie — als Langzeitbehandlung fortgesetzt — scheint einen verstärkenden und verlängernden Einfluß auf die guten klinischen Behandlungserfolge mit Venoruton® zu haben.
