Summary
The aim of this prospective, randomized, double-blind and controlled trial was to investigate the effect of O-(β-hydroxyethyl)-rutosides (HR) on plethysmographic findings, calf circumference and subjective symptoms in patients with chronic venous insufficiency. 149 patients were recruited and 143 remained for analysis (71 HR und 72 placebo). HR was administered as an initial intravenous injection of 1,000 mg followed by oral treatment (500 mg × 3) for four weeks. Reserve venous volume, venous emptying and capillary filtration rate were measured by strain gauge occlusion plethysmography. Calf circumference, subjective symptoms and side effects were recorded. After the i. v. injection of HR there were significant increases in reserve venous volume and decreases in capillary filtration rate in various sub-groups. After four weeks of oral treatment reserve venous volume was again found to be increased as well as venous emptying in some subgroups but capillary filtration rate was not altered. In female patients with oedema calf circumference was significantly reduced along with the subjective symptom of swollen leg.
Zusammenfassung
Ziel der vorliegenden Untersuchung war es, den Einfluß von O-(β-Hydroxyethyl)- rutoside auf die plethysmographischen Werte, Wadenumfang und die subjektiven Symptome bei Patienten mit chronisch-venöser Insuffizienz zu untersuchen, die weitgehend anhand eines Fragebogens der örtlichen Gesundheitsbehörden ausgewählt wurden, mit dessen Hilfe die Prävalenz der Venenerkrankungen im Gebiet von Skövde ermittelt werden sollte.
Ausgewählt wurden für den Versuch 149 Patienten, die Auswertung erfolgte für 143 Patienten (71 HR und 72 Plazebo). Es handelte sich um einen prospektiven, randomisierten Doppelblindversuch mit Plazebokontrolle. HR wurde intravenös als einmalige Injektion von 1000 mg verabreicht, gefolgt von einer oralen Verabreichung von täglich 3 500-mg-Tabletten über 4 Wochen.
Folgende Parameter wurden vor, und 60 min nach der intravenösen Gabe, sowie nach der 4 wöchigen oralen Behandlung bestimmt: Reservevenenblutvolumen (RVV), Venenentleerung (VE), Kapillarfiltrationsrate (CFR) durch „strain-gauge“- Verschlußplethysmographie, Wadenumfang, subjektive Symptome und Nebenwirkungen.
Nach der i. v. Injektion von HR kam es bei verschiedenen Untergruppen zu signifikanten RVV-Zunahmen und CFR-Abnahmen, während die VE-Werte unverändert blieben. Nach 4 wöchiger oraler Behandlung waren die RVV bei 5 verschiedenen Untergruppen wiederum erhöht, ferner die VE bei Patienten, die zu drei dieser Untergruppen zählten; die CFR hingegen zeigte keine signifikante Veränderung. Bei den weiblichen Patienten mit Ödem waren sowohl der Wadenumfang wie auch die subjektiven Symptome des „geschwollenen Beines“signifikant reduziert. Für die anderen Symptome oder die Nebenwirkungen wurden keine Differenzen nachgewiesen.
