Zusammenfassung
Für eine detaillierte Darstellung der allgemeinen und speziellen Grundlagen der Diätbehandlung von Dünndarmerkrankungen wird auf die neueren monographischen Darstellungen von Kasper (1978, 1980) sowie von Floch (1981) hingewiesen. Eine rationale Diättherapie hat von der Kenntnis der Pathophysiologie der zu behandelnden Krankheiten auszugehen. Eine kausale Therapie durch diätetische Maßnahmen ist nur bei Erkrankungen möglich, bei denen Nahrungsbestandteile selbst die pathologischen Veränderungen am Dünndarm hervorrufen. Die Elimination dieser Substanzen aus der Nahrung beseitigt die auslösende Ursache der Erkrankung. Lediglich die glutensensitive Enteropathie (Zöliakie oder einheimische Sprue) und die seltenen intestinalen Nahrungsmittelallergien, bei denen Nahrungsallergene immunologische Reaktionen am Dünndarm auslösen, sind einer kausalen Diättherapie zugänglich. Häufiger haben diätetische Maßnahmen das Ziel, durch qualitative oder quantitative Veränderungen in der Nahrungszufuhr krankheitsbedingten Funktionsausfällen und deren Folgen zu begegnen. Werden bestimmte Nahrungssubstanzen nicht resorbiert und löst ihr Verweilen im Darmlumen Beschwerden (z.B. Durchfälle) oder pathologische Folgen (z.B. Elektrolytverlust) aus, so können diese Krankheitssymptome durch Weglassen dieser Nahrungsbestandteile behandelt werden. Dieses Prinzip liegt der Diät bei allen Formen der Kohlenhydrat- oder Fettmalabsorption zugrunde.
