Zusammenfassung

Seit der Erstbeschreibung des M. Crohn durch Crohn, Ginzburg und Oppenheimer im Jahre 1932 hat sich das Wissen über die Krankheit vermehrt, die Ursache ist jedoch immer noch nicht bekannt. In den verschiedenen Namen spiegeln sich die sich wandelnden Auffassungen wider. Crohn etal. (1932) beschrieben als Entität eine Ileitis regionalis, wodurch der segmentäre Charakter und die Lokalisation im Ileum angesprochen wurden. Weil besonders häufig das letzte Segment des Dünndarms befallen ist, wurde der Begriff Ileitis terminalis lange Zeit bevorzugt. Beide Bezeichnungen sind jedoch etwas eng und verleiten zu der irrigen Annahme, daß die Lokalisation nur im Ileum zu suchen sei. Während der Befall im Kolon durch die Bezeichnung Colitis granulomatosa ebenfalls nur die Abgrenzung zur Colitis ulcerosa hervorhebt, lassen 2 weitere Namen der Krankheit, Enteritis granulomatosa und Enteritis regionalis mehr den wahren Charakter der Krankheit erkennen. Da aber auch aus ihnen die Tendenz der Krankheit, den gesamten Gastrointestinaltrakt zu befallen, nicht entnommen werden kann, bleibt der Name Morbus Crohn, der sich vom Erstbeschreiber ableitet, die treffendste, weil am wenigsten kompromittierende Bezeichnung. Korrespondierend dazu wird die Krankheit im angloamerikanischen Raum meist als Crohn’s disease geführt. Ursprünglich galt die Darmtuberkulose als wichtig bei der Abgrenzung zum M. Crohn (Crohn etal. 1932). Erst viel später wurde auch die Beteiligung des Kolons erkannt und entsprechend gewürdigt. Damit wurde die Abgrenzung gegenüber der Colitis ulcerosa in den Vordergrund differentialdiagnostischer Überlegungen gerückt (Lockhart-Mummery u. Morson 1960).
