Zusammenfassung
Durch schlagartige Blähung eines intraaortalen Ballonkatheters unmittelbar nach Schluß der Aortenklappe wird das vor und in dem Ballonbereich liegende Blutvolumen während der Diastole nach zentral verschoben. Dabei kommt es zu einer den systolischen Druck übersteigenden Erhöhung des diastolischen Druckes in der Aortenwurzel. Diese diastolische Augmentation führt zu einer Verbesserung der Koronardurchblutung im linken und rechten Ventrikel (Abb. V-l, Abb. V-2). Durch phasengerechte rasche Entleerung des Ballons kurz vor Beginn der Systole wird der Blutstrom in die Aorta freigegeben und dabei ein Volumendefizit von der Größe des Ballonvolumens (30–50 ml bei Erwachsenen) erzeugt. Hierdurch kommt es zu einer Senkung des systolischen arteriellen Druckes, womit eine Abnahme der systolischen Wandspannung und der Auswurfimpedanz sowie eine Senkung des linksventrikulären Sauerstoffbedarfes verbunden ist. Aus der Steigerung des O2-Angebotes und der gleichzeitigen Verminderung des O2- Bedarfes ergibt sich eine Verbesserung der Energiebilanz des Herzens, die in einer Senkung pathologischer Füllungsdrucke im linken und rechten Ventrikel zum Ausdruck kommt. Mit der Abnahme der diastolischen Wandspannung ist sekundär eine Zunahme der subendokardialen Durchblutung und eine zusätzliche Senkung des links- und rechtsventrikulären O2-Bedarfes verbunden.
