Zusammenfassung

Herzrhythmusstörungen bleiben eine Erkrankung von wesentlicher Bedeutung für die Gesundheit der Bevölkerung und ihre Therapie eine Herausforderung für den Arzt. Die Probleme in Diagnose und Therapie sind zahlreich, und groß war und ist deshalb der Wunsch nach einer rationalen Basis für die Therapie. In den letzten 20 Jahren ist die Zahl jener Arbeiten rapide angewachsen, welche sich mit der Entstehung von Herzrhythmusstörungen und mit der Wirkungsweise von Antiarrhythmika auseinandersetzen. Weiter konnten vor 20 Jahren fast 90% der heute möglichen Verordnungen nicht ausgeführt werden, weil das Angebot an Arzneimitteln sehr viel kleiner war. Beide raschen Entwicklungen können für die Therapie nur dann echte Vorteile bringen, wenn sie auch zur Verbreiterung der rationalen Basis der Therapie mit Antiarrhythmika beitragen. In der Therapie mit Antiarrhythmika haben deshalb neben der subjektiven Erleichterung des Patienten und der klinisch objektivierbaren Besserung des Befundes experimentelle Ergebnisse mit pharmakologischen, physiologischen und biophysikalischen Methoden das besondere Interesse des Klinikers gefunden. Molekulare und zelluläre Effekte werden bevorzugt herangezogen, um die biologische Wirkung bzw. die recht unterschiedlichen Wirkungsspektren der Antiarrhythmika zu beschreiben und klinische Ergebnisse theoretisch zu untermauern.
