Zusammenfassung

Zur Volumenersatz-Therapie empfehlen wir die Verwendung von hochmolekularer Dextranlösung, weil

  1. a.

    Gelatine und Stärke, im Gegensatz zu Dextran, keinen antithrombotischen Effekt besitzen

  2. b.

    Gelatine geringer volumenwirksam ist und

  3. c.

    Gelatine und Stärke ebenso häufig wie Dextran anaphylaktoide Reaktionen auslösen können (Inzidenz 0,1–0,2%), jedoch nur bei Dextran eine spezifische Ausschaltung dieser Nebenwirkungen möglich ist (vorherige intravenöse Injektion von 20 ml 15% Haptendextran [Promit]).
