Zusammenfassung
Der Schwerbrandverletzte ist im Hinblick auf die Anästhesie stets als Risikopatient einzustufen. Gefährdungen ergeben sich einerseits aus der universellen Schädigung des Organismus, andererseits aus technischen Besonderheiten, bedingt durch Lokalisation der Verbrennung sowie Art der operativen Behandlung. In der Primärphase, also den ersten 36 – 48 h, gilt es zunächst, durch eine der Verbrennung angepaßte Schocktherapie akut das Überleben zu sichern. Eine Narkose während ausgeprägt instabiler hämodynamischer Verhältnisse ist kontraindiziert, es seidenn, lebensbedrohliche Begleitverletzungen zwingen notfallmäßig zu einer Anästhesie. Ein mögliches Vorgehen für diese Situation ist in der Tabelle 1 skizziert. Die Anwendung von Succinylcholin ist zu diesem frühen Zeitpunkt noch ungefährlich. Läßt sich ein voller Magen nicht mit Sicherheit ausschließen, so hat die Induktion als “Sturzeinleitung” oder die Intubation am wachen Patienten zu erfolgen.
