Zusammenfassung

Über 25 Jahre ist es her, seit Lillehey die erste erfolgreiche Korrektur einer Fallot-Tetralogie gelang [41]. Kirklin zeigte wenige Jahre später, daß die Korrektur mit niedrigem Operationsrisiko vorgenommen werden kann [32]. Inzwischen wurden weitere entscheidende Fortschritte gemacht. Trotzdem besteht auch heute Uneinigkeit darüber, wann der operative Eingriff indiziert ist und welches Vorgehen gewählt werden sollte — das zweiphasige Vorgehen, bestehend in einer palliativen aortopulmonalen Anastomose zur Verbesserung der Lungendurchblutung und späterer Totalkorrektur, oder die primäre Korrektur. Welches Verfahren auch immer vorgezogen wird, das chirurgische Vorgehen spielt bei der Fallot-Tetralogie eine entscheidende Rolle. Denn ohne Operation hat dieser Herzfehler eine sehr schlechte Prognose: Ein Drittel der Patienten verstirbt im ersten Lebensjahr, die Hälfte bis zum dritten Lebensjahr und nur ein Viertel der Patienten überlebt 10 Jahre [6].
