Zusammenfassung
Eine Vollnarkose durch Inhalationsanaesthetika oder Neuroleptanaesthesie ist mit starken Veränderungen der Nierenfunktion verbunden [1–3]. Renaler Blutfluß, glomeruläre Filtrationsrate, Elektrolyt- und Wasserausscheidung werden herabgesetzt (Tabellen 1 u. 2). Zu den Mechanismen, die für diese Veränderungen verantwortlich sind, zählen Verminderung des Herzzeitvolumens und des arteriellen Blutdrucks, erhöhte sympathikoadrenale Aktivität, Freisetzung von antidiuretischem Hormon (ADH) und möglicherweise ein Aldosteroneffekt auf die Tubuli nach Aktivierung des Renin-Angiotensin-Systems. Normalerweise ist aber die Nierenfunktion innerhalb weniger Stunden nach Beendigung von Narkose und Operation wieder voll hergestellt, mit Ausnahme der freien Wasserausscheidung, die oft für mehrere Tage — wahrscheinlich durch eine andauernde, hohe ADH-Aktivität — beeinträchtigt ist [4].
