Zusammenfassung

Bereits unter physiologischen Bedingungen reguliert der obere Dünndarm wechselnd große und in Osmolarität sowie pH unterschiedlichste Flüssigkeitsvolumina: ca. 1 1 Sekret aus den Speicheldrüsen (Davenport 1966), das im Vergleich zur Körpertonizität osmotisch stark hypoton ist (Thaysen et al. 1954), ca. 2 1 Magensaft mit überwiegend sauren Valenzen (Davenport 1966), ca. 3 1 Sekret aus exokriner Pankreassekretion und biliärer Sekretion (Doubilet u. Fishman 1961) sowie die Sekretmenge, die von der Schleimhaut des oberen Dünndarms, speziell den Brunner-Drüsen als succus entericus produziert wird und mit ca. 1 1/die (Phillips 1972) kalkuliert werden kann. Hinzu kommen ca. 2 1 Flüssigkeit durch Nahrungsaufnahme, wobei hier Extreme bezüglich Volumen, Gesamtosmolarität, Elektrolyt- und Säure-Basen-Gehalt zu äquilibrieren sind. Berücksichtigt man weiter, daß die Nahrung selbst sowie insbesondere die Spaltprodukte der Kohlenhydrat- und Eiweißverdauung im Lumen des oberen Dünndarms als osmotisch aktive Substanzen zu kompensieren sind, so wird klar, welche Bedeutung dem oberen Dünndarm allein für die Aufrechterhaltung der Homöostase unter physiologischen Bedingungen zukommt.
