β-Blockade bei Angina pectoris

Zusammenfassung

Die Angina pectoris signalisiert eine zeitlich und örtlich begrenzte Bilanzstörung zwischen myokardialem Sauerstoffbedarf und Sauerstoffangebot, der in der überwiegenden Mehrzahl der Fälle eine organisch fixierte, druck- und durchflußmindernde Stenose der großen Koronargefäße oder ein kollateral überbrückter Koronarverschluß zugrunde liegen. Abgesehen von der seltenen Form der erstmals von Prinzmetal [17] beschriebenen vasospastischen Angina und der funktionellen Koronarinsuffizienz bei kritischer Widerstandshypertrophie des linken Ventrikels infolge maligner arterieller Hypertonie, hypertropher Kardiomyopathie oder Aortenklappenfehlern ist die Koronarinsuffizienz ein gefäßmechanisches Problem.
