Abstract
To evaluate the safety of a β-Propiolactone/Ultraviolet (BPL/UV), irradiated Factor IX complex preparation we inoculated eight chimpanzees with 25 units Factor IX/Kilo from a pool of five production lots which had been treated in this manner. These lots were derived from approximately 1,000 donors. Animals were followed with weekly tests for hepatitis B serologic markers and transaminases, and biweekly liver biopsies, for 6 months. No evidence of transmission of hepatitis B, or non-A, non-B viruses was observed.
To further evaluate the BPL/UV procedure a plasma pool was intentionally contaminated with hepatitis B virus and one half of the pool treated with BPL/UV. Factor IX complex was isolated from the treated and untreated pools and each was inoculated into four chimpanzees. The Factor IX derived from untreated plasma infected all four animals with an average incubation period of 10.5 weeks, whereas that prepared from BPL/UV treated plasma infected only one of four animals with an incubation period of 21 weeks. These results were interpreted as suggesting that BPL/UV can inactivate approximately 99.9% of hepatitis B virus infectivity.
Zusammenfassung
Gerinnungsfaktoren gehören zu den high risk-Produkten bezüglich der Hepatitisübertragung. Eine Möglichkeit, dieses Risiko zu senken, besteht in der Sterilisation von Plasma mit β-Propiolacton/UV (β-PL/UV) (LoGrippo 1956). Da kontrollierte klinische Studien schwierig bzw. unmöglich zu erbringen sind, wurde PPSB aus β-PL/UV sterilisiertem Plasma am Schimpansen auf Infektiosität geprüft.
Acht jugendlichen Schimpansen wurde eine Mischung aus fünf PPSB-Handelschargen in einer Dosierung von 25 E/kg KG intravenös verabreicht. In dem folgenden Beobachtungszeitraum von sechs Monaten wurden die Schimpansen wöchentlich auf erhöhte Transaminasen, HBsAg, anti-HBs und alle vier Wochen auf anti-HBc untersucht. Zusätzlich wurde alle 14 Tage eine Leberbiopsie durchgeführt. Es wurden nach Auswertung aller Ergebnisse keine Hinweise auf die Übertragung einer Hepatitis B oder einer Hepatitis non A — non B gefunden. Die Suszeptibilität der Tiere wurde in der anschließenden Untersuchung festgestellt.
Zur quantitativen Abschätzung des Wirkungsgrades der β-PL/UV-Behandlung wurde ein Plasma-Pool vorsätzlich mit infektiösem Plasma (HBsAg-, HBeAg-, DNA-Polymerase-positiv, Dane-Partikel enthaltend) kontaminiert und eine Hälfte dieses Pools mit β-PL/UV behandelt. Sowohl aus dem unbehandelten Teil des Pools, als auch aus dem behandelten Teil wurden PPSB-Testfraktionen hergestellt. Das so erhaltene nicht sterilisierte PPSB-Konzentrat aus dem unbehandelten Pool wurde vier Schimpansen aus dem ersten Teil der Untersuchung verabreicht und verursachte bei allen vier Tieren eine Hepatitis B mit einer Inkubationszeit von 10,5 Wochen, was einer Infektiosität von ca. 103 CID50 entspricht.
Die aus dem mit β-PL/UV behandelten Teil des Plasmas hergestellte PPSB-Fraktion wurde vier weiteren Schimpansen verabreicht. Drei der Tiere zeigten keinerlei Anzeichen einer Hepatitis B-Infektion. Ein Tier entwickelte nach 21 Wochen eine Hepatitis B. Die HBV-Infektiosität wurde somit um ca. 103 CID50 auf < 1 CID50 gesenkt, was einem Faktor von > 103 entspricht.
