Zusammenfassung
In den letzten Jahren haben immunologische Fragen zunehmendes Interesse in der Anaesthe-sie gewonnen. Nach größeren Eingriffen ist die Immunreaktion für mindestens eine Woche unterdrückt, dies hat vermutlich bei jüngeren Patienten keine pathologische Bedeutung. Bei Patienten gleichen Alters zeigte sich nach Operationen eine Abhängigkeit der Überlebenschance vom Funktionszustand des Immunsystems. Aus eigenen Untersuchungen des RES könnte man annehmen, daß eine länger dauernde Immunsuppression die Metastasierung von Tumorzellen in Zusammenhang mit chirurgischen Eingriffen begünstigt [16]. Es gibt, wie später aufgezeigt wird, sogar Hinweise dafür, daß die Immunreaktion durch unterschiedliche Narkoseverfahren verschieden stark beeinflußt wird. Andererseits wird diese Wirkung durch den Operationsstreß überlagert, der, per se, eine Immunsuppression bewirkt. Aus Tierversuchen ist bekannt [29], daß eine Halothanenarkose die Toxizität von Bakterien und Viren erhöht. Beobachtungen über den Einfluß einer Halothanenarkose (ohne Operation) liegen bisher am Menschen nicht vor. Auch wenn eine wesentliche Streßreaktion auf eine reine Halothanenarkose (ohne Operation) vermutlich verneint werden kann, so darf jede immunologische Fragestellung nur im Zusammenhang mit laborchemischen Streßparametern beantwortet werden.
