Abstract

The positive inotropic actions of the cardiotonic steroids and their glycosides can be distinguished from those of other positive inotropic agents, such as catecholamines or xanthine derivatives (see Chap. 10 and 11). The differences in the inotropic actions of these substances suggest differences in their mechanisms of action. Several substances, however, possess positive inotropic properties similar to those of cardiac glycosides. Comparative studies on these glycosides and aglycones indicate strict structural requirements for their cardiotonic actions (see Chap. 2). Considering that only a slight modification of the chemical structure results in a marked reduction in the inotropic potency, it is quite remarkable that substances of different chemical structure (Fig. 1) exhibit positive inotropic actions quite similar to those of cardiac glycosides. Some of these compounds inhibit, as does digitalis, cardiac Na+, K+-ATPase or the sodium pump, whereas others have no direct effect on this enzyme system. The relationships between effects of these substances on Na+, K+-ATPase activity, sodium pump activity, or transmembrane sodium fluxes and the force of myocardial contraction merit consideration.
