Influence of Cardiac Glycosides on Myocardial Energy Metabolism

Abstract

According to our present knowledge, myocardial energy metabolism is not involved directly in the mode of action of cardiac glycosides. However, it is well known clinically that the therapeutic efficiency of these drugs is to a certain degree dependent on the metabolic state of the heart; they are most effective in hearts with no defects in energy metabolism such as congestive heart failure due to excessive hemodynamic pressure loads (e.g., hypertension, valvular lesions, arteriosclerotic diseases) whereas they are rather ineffective in cardiac failure due to anemia, hypoxia, thyrotoxicosis, and severe thiamine deficiency (beriberi) in which energy supply is impaired (Moe and Farah, 1975) and experimentally it has been shown that heart failure caused by poisons that reduce energy-rich phosphate pools (cyanide, dinitrophenol, etc.) is not relieved by digitalis (Gruiizit and Farah, 1955).
