Abstract
Pauwels showed many years ago that an inter-trochanteric osteotomy can change the loading of a hip, and demonstrated the profound effects that such a change in loading can bring about. A full discussion of this subject can be found in the chapters here by Pauwels, Bombelli, and Schneider. These biomechanical principles form a basis for the understanding of the effects of an intertrochanteric osteotomy in the treatment of pseudarthrosis of the femoral neck, coxa valga with subluxation, and osteoarthritis.
