Zusammenfassung
Die zirkuläre endokardiale Ventrikulotomie (ZEV) wird eingesetzt zur chirurgischen Behandlung chronischer ventrikulärer Tachykardien (VT), die nach einem Myokardinfarkt (MI) auftreten. Diese Technik basiert auf der pathophysiologischen Bedeutung einer Infarktnarbe. Die Läsionen eines Myokardinfarktes bieten 3 bemerkenswerte Beobachtungen [19]: Die Myokardnekrose ist maximal im Zentrum des Infarktareals. Diese Zone fibrosiert und wird dünnwandig. Ein Mapping der ventrikulären Erregung zeigt keine elektrische Aktivität über dieser Zone, so daß sie keine Rolle für die Genese der Arrhythmien spielen kann. Um die zentrale Nekrosezone ist die Grenzzone gelegen, deren Verlauf schwierig auszumachen ist. Die Grenzzone besteht aus ischämischem und normalem Myokard sowie Fibröse. Die ischämischen Läsionen sind im wesentlichen in den subendokardialen Schichten lokalisiert. Eine Endokardfibrose entwickelt sich im Bereich des gesamten Infarktareals. Die Ausdehnungen eines pathologischen Areals können daher durch endokavitäre Exploration lokalisiert werden. Die Grenze der Endokardfibrose entspricht der maximalen Ausdehnung der Läsionen.
