Zusammenfassung
Metoprolol ist ein wirksamer kardioselektiver Blocker der adrenergen Betarezeptoren ohne symphatikomimetische Eigenwirkung („intrinsic activity“), ohne Wirkung auf die Gefäßperipherie und auf den Bronchustonus [1, 9]. Nach ersten Berichten zeigt Metoprolol, i.v. verabreicht, auch günstige Effekte bei verschiedenen Tachyarrhythmien, vor allem supraventrikulären Ursprungs [6, 9]. Elektrophysiologische Untersuchungen am Menschen zeigten eine Verlängerung der effektiven Refraktärperiode des AV-Knotens und ließen so günstige Wirkungen bei Reentry-Tachykardien mit Beteiligung des AV-Knotens erwarten [7]. Wir untersuchten deshalb die Wirkung von i.v. verabreichtem Metoprolol an 14 Patienten, bei denen Reentry-Tachykardien reproduzierbar durch programmierte Stimulation des Herzens ausgelöst werden konnten, und an weiteren 20 Patienten bei chronisch oraler Behandlung.
