Zusammenfassung
Der Morbus Crohn ist eine chronisch entzündliche Darmerkrankung unbekannter Ätiologie und seine Therapie bleibt deshalb auf eine unspezifische Unterdrückung des Entzündungsprozesses beschränkt. Versagen konventionelle Therapiemaßnahmen, wie Diät, Ruhe, Corticosteroide, Azulfldine und Immunsuppressiva im akuten Schub oder treten Komplikationen wie Ileus, Blutung oder Fisteln auf, bedarf es chirurgischer Maßnahmen, um den Patienten über einen längeren Zeitraum hinweg von seinem Leiden zu befreien. Die Möglichkeiten der chirurgischen Therapie sind jedoch bei einem multilokalären Darmbefall beschränkt. Zudem muß 10 Jahre nach der Operation mit einer Rezidivrate bis zu 80% gerechnet werden (Tabelle 1). Die Wahrscheinlichkeit für einen neuen Eingriff beträgt dann 50% [6]. Liegt die Letalität beim ersten Eingriff noch zwischen 2–6%, so kann sie bei jedem Folgeeingriff bis zu 50% ansteigen [3]. Hieraus ergibt sich klar, daß Maßnahmen sinnvoll sind, die das chirurgische Vorgehen aufschieben oder verhindern können, da das Resultat wiederholter Eingriffe an den bei der Erstmanifestation meist jugendlichen Patienten ein sogenann-tes Short-Bowel-Syndrom sein kann.
