Zusammenfassung
Die chirurgische Behandlung der Ileocolitis granulomatosa Crohn und der Colitis ulcerosa ist durch die noch ungeklärte Ätiopathogenese auf die Beseitigung konservativ nicht beherrschbarer Komplikationen und therapierefraktärer Verläufe beschränkt [4, 8]. Operationsindikationen wie -verfahren sind mit der Notwendigkeit streng individualisierenden Vorgehens allgemein akzeptiert (Tabelle 1 u. 2). Abgesehen von chirurgischen Notfällen des toxischen Megakolons und der Perforation sollten diese Erkrankungen wegen des hohen operativen Risikos durch konservative Maßnahmen möglichst in die Remission überfuhrt werden. So kann die Elektivoperation mit vertretbarem Mortalitäts- und Morbiditätsrisiko durchgeführt werden.
