Zusammenfassung
Die pathogenetische Bedeutung des Gastrins beim „banalen“ Ulcus duodeni ist bis heute nicht geklärt. Gesichert ist eine gesteigerte Magensäuresekretion unter basalen und verschiedenen Stimulationsbedingungen, die mit einer gesteigerten Parietalzellmasse und einer erhöhten Serumpepsionogenkonzentration, nicht jedoch mit dem basalen Serumgastrin korreliert [28]. Basale Serumgastrinkonzentrationen liegen nach den Angaben der meisten Autoren und nach den eigenen Untersuchungen im Normbereich (Abb. 1). Dagegen ist der mittlere postprandiale Gastrinanstieg bei Patienten mit Ulcus duodeni signifikant höher als bei Kontrollpersonen [18]. Dies wird auf eine höhere funktionelle Aktivität des Antrum zurückgeführt, da sich die Anzahl der antralen G-Zellen des Duodenalulcus-Patienten nicht signifikant von denen gesunder Probanden unterscheidet [6].
