Zusammenfassung

Medikamente passieren die Niere in einer Sequenz unidirektio-naler (Filtration) und gegenläufiger Transportprozesse (tubuläre Reabsorption und Sekretion), Richtung und Ablauf dieses Stofftransports werden allerdings durch die pharmakologische Struktur der Einzelsubstanz näher bestimmt. Meist haben Medikamente ein niedriges Molekulargewicht (< 500) und werden — dissoziiert oder undissoziiert und abhängig von ihrer Proteinbindung — filtriert. Wasserlösliche Stoffe und Ionen werden aktiv reabsorbiert (z.B. Aminosäuren) oder mit dem Harn direkt ausgeschieden; lipoidlösliche Stoffe dagegen gelangen durch sie sog. nonionic diffusion ins peritubuläre Plasma zurück und werden erst durch die hepatische Biotransformation als polarere Substanzen nierengängig.
