Abstract

Deformities of this nature (synonyms: Spalthand, split hand, lobster-claw hand) seem to be caused by a wedge-shaped defect of the soft tissue blastema of the primitive hand bud (W. Müller 1937). An ectodermal defect has also been considered (Werthemann 1952). Skeletal development is apparently secondarily suppressed in a wedge-shaped configuration with the basis of the wedge located peripherally. Its depth, width, and direction show marked differences. If there is a small wedge-shaped defect, the center of its base is located at the tip of the IIIrd ray and/or its adjacent interdigital spaces, taking the normal hand as a baseline. Major involvement of the radial side, which is formed during a later stage of biogenesis, results in wider defects.
