Bisherige Erfahrungen zur operativen Behandlung der Hyperhidrosis axillaris

Summary

The surgical technique by means of the oval incision or its modification (Y-incision, semicircular incision) represents the radical method of removing the Hyperhidrosis axillaris by total excision of the sweating region. The recently propagated variants of therapy (currettage, Schick-Dermatom-Technique) could lead to better cosmetical results, but the actual experience shows that the radical elemination of the sweating area cannot be effected thoroughly.

By use of a hard resorbable plastic thread (Vicryl) in practising the excision technique we are able to reduce the scarformation to a minimum.

Zusammenfassung

Die Operationstechnik mittels Ovalärschnitt oder dessen Modifikationen (Y-Schnitt, halbkreisförmige Schnittführung) stellt als totale Exzision des schwitzenden Areals die radikalste Methode zur Beseitigung der Hyperhidrosis axillaris dar. Die neuerdings propagierten Behandlungsvarianten (Kürettage, Schick-Dermatom-Technik) könnten zu einem kosmetisch besseren Resultat führen, das aber nach den bisherigen Erfahrungen zu Lasten der Radikalität geht. Durch Verwendung eines schwer resorbierbaren Kunststoff-Fadens (Vicryl) konnte in unserer Klinik die Narbenbildung bei der Exzisionstechnik auf ein Minimum reduziert werden.
