Summary
In the primary treatment of malignant melanoma of the skin the immediate elective discontinuous node dissection offers not better prognosis than does a therapeutic dissection. Indeed for patients with excision only of the primary melanoma, follow-up examination must be done once a month, which is difficult due to long term surveillance. In this connection an inguinal skin excision for lymph node dissection is described which minimizes wound complications in the postoperative period. The main factor in wound complications is due to dissection of the vascular architecture. The wide spindle-S-shaped excision prevents the later devascularisation of skin flaps. If malignant melanoma is in an anatomical position which lends itself to incontinuity dissection, removing primary tumor, intervening lymphatics and regional lymph nodes en bloc can be combined with the incision lines. Furthermore the excised groin skin can be used for a free flap to cover the excision wound of the primary tumor.
Zusammenfassung
Nach neueren statistischen Erhebungen bietet beim malignen Melanom der Haut, Stadium I, die einzeitige prophylaktische diskontinuierliche Lymphknotenektomie regionaler Lymphknotengruppen keine Gewähr für bessere Überlebenschancen als die zweizeitige kurative Methode, bei der zunächst der Primärtumor weitgreifend entfernt wird und die regionalen Lymphknotengruppen bei einem evtl. späteren metastatischen Befall extirpiert werden. Letzteres Vorgehen erfordert jedoch eine engmaschige Überwachung der Kranken über Jahre, deren Durchführungsmöglichkeit problematisch erscheint. In diesem Zusammenhang wird eine Hautschnittführung zur Ektomie der Leistenlymphknoten aufgezeigt, durch die postoperative Wundheilungsstörungen vermieden werden können, da dabei das durch operationsbedingte Devaskularisation in seiner Trophik gefährdete Hautareal beseitigt wird. Die Hautschnittführung eignet sich auch zur Ausführung der Kontinuitätsdissektion, bei der Primärtumor, ableitende Lymphbahnen und regionale Lymphknotengruppen einzeitig entfernt werden.
